Background

AIM CCI’s Community Care to Address Management of Chronic Conditions during Pregnancy Bundle seeks to assure that all women/birthing persons affected by diabetes, hypertension, and overweight/obesity have equitable access to recommended preventive services, primary and specialty care that is congruent with their needs during pregnancy.

The prevalence of Severe Maternal Morbidity (SMM) can be influenced by various factors, including chronic conditions such as diabetes, hypertension, and overweight / obesity. Left unmanaged, these conditions can lead to maternal mortality, lessen the quality of care, and exacerbate disparities in maternal health outcomes. Recognizing the importance of addressing chronic conditions during pregnancy and the postpartum period is critical to improving maternal health outcomes.
